Supported Independent Living (SIL)

  • Support Coordination is a funded support under the NDIS that helps participants make sense of their plan, connect with the right services, and get the supports they need to reach their goals. It’s not about providing direct care (that’s what support workers do) — it’s about guiding, organising, managing, and ensuring all the moving parts of your support plan work together.

    At ProCare, Trusted Support Coordination means we don’t just tick boxes. We take the time to understand your life, your routines, and your mental & emotional health, and build a tailored team (from support workers through to therapists, allied health, OTs, GPs, etc.) so your plan feels seamless, helpful, and stable.

    There are different levels of Support Coordination under the NDIS:

    • Support Connection – more basic guidance and linking you with providers/community supports.

    • Coordination of Supports – more comprehensive, managing multiple providers and ensuring everything works together.

    • Specialist Support Coordination – for those with complex needs or barriers (for example mental health, behaviour needs, high risk) requiring higher oversight and coordination.

  • A trusted support coordinator (or independent support coordinator) at ProCare helps:

    • New Plan Recipients who have just got their NDIS plan and are unsure how to begin, what the different supports mean, or how to use their funding.

    • Participants with multiple supports/services (therapists, carers, housing, social supports, medical etc.) who are overwhelmed with managing providers, schedules, and making sure everything works together.

    • Families or carers who are overwhelmed by the funding, the administration, the paperwork, trying to understand what supports are reasonable and necessary, and who just want someone to reliably take care of coordination so they can get back to caring and living.

    • Anyone seeking greater independence — who wants to maximise their NDIS plan to achieve goals (daily living, social connection, health, mental wellbeing etc.), reduce time wasted, reduce the number of carers switching out, increase consistency, and get value for what’s funded.

    When you work with an independent support coordinator like ProCare you get someone whose only job is you, not doing support work plus admin, someone who advocates, guides, and holds things together.

  • Here’s what a ProCare Support Coordinator will typically do with you:

    1. Plan Understanding & Goal Setting
      We’ll sit with you (or your family/carer) to deeply understand your NDIS plan: your budget, your goals, your priorities. We ask: what matters most to you? Where are the gaps? What supports are underused or misused?

    2. Service & Provider Matching
      We identify and vet service providers, allied health, specialists, therapists, social supports, and any mainstream services relevant. We help you choose providers who align with your values, schedule, location, culture, and level of trust.

    3. Coordination & Scheduling
      We manage the complexity: schedule appointments, ensure service agreements are in place, follow up, deal with no-shows, and help you avoid rotating carers so much. We make your supports predictable and stable.

    4. Capacity Building
      We help you build skills so you’re more confident working with providers, managing portions of your plan, making reviews less scary, and ultimately being more independent.

    5. Problem Solving & Advocacy
      When things break down — provider issues, funding questions, unexpected health or mental health needs — we step in. We liaise with NDIA, providers, families to fix issues or escalate when needed.

    6. Review & Adjust
      As your needs change (e.g. mental health changes, routines change, more goals appear, etc.), or as you get feedback, we adjust your supports. We help prepare for plan reviews, ensure your plan is delivering outcomes, monitor progress.

    7. Reporting and Transparency
      We keep you informed: how your supports and funds are being used, what’s working, what’s not. You always know what’s going on. We maintain records, provide transparent accounting of support coordination work, so there are no surprises.

  • Why do I need a support coordinator? Can't I just find carers / services myself?
    Yes, you can. But what most people don’t realise is how much time, energy, stress and money gets wasted when you don’t have someone coordinating. Carers who don't understand routines, scheduling conflicts, providers not communicating, funds not being used well — a good support coordinator reduces all of that, and builds consistency & trust.

    Is support coordination costly? Will it impact my budget?
    Support Coordination is funded in many NDIS plans under Capacity‑Building. If it’s in your plan, you’re not paying extra out of pocket. If it’s not, sometimes people pay fee‑for‑service or request it in their plan. At ProCare we are explicit and transparent: how many hours we expect to spend on coordination, what the outcome is. We aim to deliver trusted support coordination that gives more value than what you pay for.

    How do I know you’ll follow through / not just change support workers or disappear?
    Our model emphasises continuity: building a core trusted team around you (not just temporary fill-ins), consistent communication, clear reporting, feedback loops. If something’s not working, we fix it. We hold ourselves accountable via regular check-ins and trusting relationships.

    How much control will I have?
    You have all control. We don’t make decisions without you. We offer options, advice, advocate on your behalf, but your goals, your preferences, your choices drive everything. That’s part of being an independent, trusted support coordinator.

    What if something in my plan changes — health, mental health, funding?
    We adapt. Part of our ongoing service is to monitor how things are going, identify when your supports are no longer sufficient or aligned, and help you with plan reviews or amendments. We ensure flexibility so your plan continues to serve you.

    3. Kickoff & Provider Matching
